New Patient Paperwork
Redmond Signature Dentistry strives to make your first visit as smooth as possible, we recommend completing your new patient paperwork in advance. You may complete our new patient paperwork before your appointment. Whether you prefer to register online or fill out the forms on paper, we have both available to you.
What to Expect During Your First Visit
We are located on the street level across from the Marriott Hotel or directly on the corner of the parking garage.
When you arrive for your new patient appointment, we will ask you to complete any remaining new patient paperwork. We ask that you please bring the following items with you to your appointment:
- Your completed new patient paperwork
- Your dental insurance, if applicable.
- A list of any medications you are currently taking
- If you have had recent X-rays taken at another dental office, please request that they be sent to us in advance.
During your initial visit, we will make an effort to get to know you and answer any questions you may have. We’ll thoroughly examine your mouth, teeth, and gums and perform a cleaning. X-rays might be taken to verify that there are no underlying problems beneath the gum line that need to be addressed.
Afterward, we will develop a personalized treatment plan based on your specific dental needs and schedule any follow-up appointments.
